Privacy notice — online booking

Magtibay-Cruza Dental Clinic (“we”) collects the information you enter on the booking form: your name, mobile number, birthdate (optional), the branch, service and time you choose, any notes and promo code, and technical data needed to prevent spam (a scrambled form of your internet address and your browser type).

Why: to schedule your appointment, confirm it and remind you by SMS, and to contact you about your dental care (for example recall check-ups). We do not sell or share your information for others' marketing.

Who can see it: only authorized clinic staff, through a system with individual logins and an audit log. Our SMS provider receives your mobile number and the message text only to deliver the SMS.

How long: booking requests are kept for as long as needed for your dental records and as required by law. If you don't become a patient, we delete the request details on request.

Your rights under the Data Privacy Act of 2012 (RA 10173): to be informed, to access and correct your data, to object, to ask for erasure, and to file a complaint with the National Privacy Commission. You can stop SMS from us any time by telling our staff.
